Experience:
Two years of experience in an acute or long-term care facility or working with Individuals who have Intellectual/Developmental Disabilities.
General ResponsibilitiesThe Program Nurse is responsible for the direct coordination of healthcare services aimed at meeting the needs of individuals served by the agency.
She/he will participate as an integral part of the Interdisciplinary team.

Is knowledgeable of resident rights and ensures an atmosphere that allows for the privacy, dignity, and well-being of all individuals served in a safe, secure environment.
Supports, cooperates with and implements specific procedures and programs for:
Safety, including standard precautions and safe work practices, established fire/safety plans, report and/or correct unsafe working conditions, equipment repair, and maintenance needs.
Confidentiality of all data, including individuals served employee and operations data.
Compliance with all regulatory requirements and agency policies as required by Human Resources, licensing, and funding agencies.
Demonstrates knowledge and compliance with HIPAA standards.
Compliance with the Code of Conduct and Reporting under the Federal Elder Justice Act.
Functions as a member of the Team:
Cooperates and works together with all co-workers; plans and completes job duties with minimal direct supervision, using appropriate judgment.
Uses tactful and appropriate communications in all situations.
Reports and follows up on reported complaints, problems, and concerns with appropriate personnel.
Promotes positive public relations with individuals served, family members, other team members, and the community.
Completes the requirements for training, acceptable attendance, dress codes including personal hygiene, and other work duties as assigned.
Completes all documentation to ensure compliance with all regulations and agency policies.
Essential Duties Utilizes the nursing process to promote optimal health of individuals served.

Develops and implements healthcare goals based on needs.

Maintains electronic health records for assigned individuals .
Prepares nursing 90-day assessment of individuals in the Older Adult program .
Prepare annual medical summaries, Health Risk Screen Tool, and other pertinent assessments .
Communicates pertinent medical information to appropriate persons, including the program coordinator, families, and day program staff.
Monitors the hygiene of individuals served and initiates corrective action as needed.
Attends specialty medical appointments as needed.
Provides health-related training to agency staff and members of the interdisciplinary team.
Ensures completion of medical appointments.
Completes pre- and post-appointment paperwork.
Obtains and transcribes physician orders.
Monitors medication distribution, storage, and disposal.
Coordinates ordering of medical equipment and supplies.
Assumes responsibility and accountability for own clinical judgment and actions.
Takes an active role in identifying ethical issues that impact care and advocates resolution.
Assists in identifying and obtaining appropriate medical care.
Provides coverage for nurse colleagues as needed.
Participates in the nursing on-call and clinic rotation.
Educates and mentors new colleagues with job tasks.
Other Duties Participates on agency committees as assigned.
Performs other work-related duties as assigned.
